From transportation to home care, Neighborhood Healthcare representatives highlighted services available through the organization’s PACE program during Tuesday’s You Matter meeting at the Menifee Library.
The presentation focused on the Program of All-Inclusive Care for the Elderly, or PACE, which serves adults 55 and older who meet the program’s eligibility requirements and are able to live safely in the community.
“It is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life,” Neighborhood Healthcare representative Stephanie Cano said.Â
Cano said each participant receives a personalized care plan created by a team of healthcare professionals. Depending on a participant’s needs, services may include primary and specialty medical care, physical and occupational therapy, prescription medications, home care, social work services, rehabilitation, meals and social activities. The program also offers behavioral health, dental and vision services.Â
Representatives said the program serves residents throughout Riverside and San Bernardino counties, including Menifee, Murrieta, Hemet, Corona, Chino, San Bernardino and Moreno Valley. Participants are assigned to either the Murrieta or Riverside PACE center based on where they live.
Participants can attend one of the program’s day centers for therapy, meals and activities. Cano said the program also provides transportation to medical appointments.
During the presentation, Cano shared the story of a participant who suffered a stroke in 2017 and later regained the ability to dress, shower and complete other daily activities independently after receiving therapy through the program. She said the participant’s experience is one of many success stories seen through PACE.Â
